Your name
Your address
Your Phone number, in case we need to contact you for additional information.
Email address.
Brief description of your sighting. Please describe what you saw, and direction of sighting, (direction of movement of the object, colour of the object, if it is a UFO sighting), the number of witnesses, besides yourself, and the duration of the sighting.
Include the date of your sighting.
The time of your sighting. Be sure to indicate what time of day it was, i.e. 8am etc.,
And the location of your sighting. i.e. Near Leeds etc.,
